equip + setup fees

I totally don't see a problem with making packages for one-time fees for        
equipment or circuit installs.  There's development scheduled in 1.9            
that will make it easier to order a bundle with those packages.                 
                                                                                
Just use a flat price plan with no recurring frequency or recurring fee.        
Put the charge in as the setup fee.
